RESUMO

Objective:To investigate the current situation of medical social worker supervision in Shanghai, for reference to promote the high-quality development of medical social work. Methods:From June to July 2022, a questionnaire survey was conducted on the in-service medical social workers in all medical institutions with medical social work departments or posts in Shanghai. The questionnaire mainly included demographic information, current status of supervision implementation, and effectiveness of supervision. The data were analyzed descriptively with t test for comparison between groups and the Pearson test was for correlation analysis. Results:A total of 99 medical social workers were included in this study, 65 had received supervision, and medical institutions where 58 people located had established the supervision system. The average scores of actual and expected supervisory support received by medical social workers were 3.71 and 4.20 respectively, and the mean burnout level score was 32.91. The effect of establishing a supervision institutions on burnout was statistically significant ( P<0.05) and actual access to supervision support was negatively associated with burnout ( P<0.05). Conclusions:The overall situation of medical social work supervision in Shanghai was positive, but the demand for supervision was not fully satisfied; Supervision had a positive impact on reducing the burnout level. It is suggested that medical social work should further strengthen the cultivation of supervisory talents, promote the construction of supervisory systems, and improve the quality of supervision.

RESUMO

@#Objective ( ) To investigate the association between urinary polycyclic aromatic hydrocarbons PAHs metabolites - Methods and high normal blood pressure in coke oven workers. A total of 433 coke oven workers were selected as the study - subjects using convenient sampling method. They were divided into normal blood pressure group and high normal blood pressure group according to their blood pressure level. The levels of ten kinds of urinary hydroxylated PAHs metabolites were measured by - Results - high performance liquid chromatography mass spectrometry. Among the subjects,57.5% had high normal blood - , - , - pressure. The levels of 1 hydroxynathalene 2 hydroxyphenanthrene 1 hydroxyphenanthrene and the metabolite of total PAHs - ( P ) in the high normal blood pressure group were higher than those in the normal blood pressure group all <0.05 . The results of - , - , - , the multivariate logistic regression analysis showed that urinary 1 hydroxynathalene 2 hydroxyfluorene 3 hydroxychrysene - ( P ), and metabolite of total PAHs were all risk factors for high normal blood pressure in coke oven workers all <0.05 after , , , , , adjusting for confounding factors such as gender length of service body mass index smoking index alcohol consumption tea , , , Conclusion consumption night shift exercise frequency and other PAHs metabolites. Exposure to PAHs in coke oven plants may increase the risk of elevated blood pressure within the normal range among coke oven workers.

RESUMO

OBJECTIVE@#To investigate the clinical characteristics of lower extremity arterial disease (LEAD) and its risk factors in patients with diabetic foot ulcer (DFU).@*METHODS@#We retrospectively collected the clinical and follow-up data of 650 patients with DFU treated in the Department of Endocrinology and Metabolism of Nanfang Hospital between January, 2017 and December, 2019. We compared the data between patients who had LEAD and those without LEAD and used a multivariate logistic regression model to analyze the risk factors of LEAD in DFU patients.@*RESULTS@#Among the 650 DFU patients, 470 (72.4%) had LEAD. The patients were followed up for a mean of 3.5 months, and the mean healing time of DFU was 2.55 months; healing of DFU occurred in 453 patients and 183 patients received amputation. The patients with LEAD and those without LEAD differed significantly in age, hospitalization costs, diastolic blood pressure (DBP), glycated hemoglobin, blood lipid levels, disease course, ankle brachial index, healing time, smoking history, clinical outcomes, Wagner grade and imaging results (P < 0.05). Multivariate logistic regression analysis identified age (OR=1.070, 95% CI: 1.049-1.091), smoking history (OR= 2.013, 95% CI: 1.268-3.195), and a decreased DBP (OR=0.980, 95% CI: 0.963-0.997) as independent risk factors for LEAD in DFU patients. A prolonged healing time was a prominent clinical feature of DFU complicated by LEAD.@*CONCLUSION@#DFU patients have a high incidence of LEAD, which leads to high rates of disability and mortality and is associated with an advanced age, high smoking rate and longer healing time. A decreased DBP is also a risk factor for LEAD in DFU patients.

RESUMO

Objective To explore the relationship between selenium and diabetic nephropathy (DN)by determining the selenium content in the plasma and whole blood of DN patients and the changes of selenoprotein expressions in human mesangial cells stimulated by high glucose. Methods We collected the samples and clinical indicators of DN patients and healthy controls to determine selenium content by atomic absorption spectrometry method. The mRNA in human mesangial cells was isolated after stimulation with 30 mmol/L of glucose for 24 hours by Trizol method to detect the changes of selenoprotein expressions by Real-time PCR.We analyzed the differences between DN patients and healthy controls and the relationship between clinical indicators by unpaired t-test and Pearson correlation analysis, respectively. Results The selenium contents in the plasma (P<0.0001)and whole blood (P<0.001)were significantly lower in DN patients than in the healthy controls. In addition, plasma selenium contents had a significant negative correlation with renal function and a positive correlation with glycosylated hemoglobin in DN patients.Among the 2 1 human selenoproteins, the mRNAs of Gpx1 ,TrxR2 ,TrxR3 ,Dio2 , Dio3 ,SelK,SelN,SelP,SelR,SelT,SelW and SPS2 were significantly lower in the high-glucose group than in the normal-glucose group which produced by human mesangial cells after high glucose stimulation for 24 hours. Conclusion There was obvious selenium deficiency in DN patients.Human mesangial cells have a significantly low expression of some selenoproteins in the high glucose environment. These results provide clinical and experimental evidence for illuminating the role of low selenium-sensitive selenoproteins in the occurrence and development of DN.

RESUMO

Objective To construct the recombinant plasmids of normal and truncated selenoprotein S genes so as to observe their biological function in vitro or in vivo. Methods We constructed the recombinant plasmids of normal and truncated selenoprotein genes by gene recombinant technology. The gene of truncated selenoprotein was coding domain sequence(CDS)fragment of mRNA;the gene of normal selenoprotein was CDS and 3'untranslated region(including Sec insertion sequence)fragment of mRNA.We confirmed the sequence of recombinant genes by sending them to a company for comparison. The recombinant plasmids of normal and truncated genes of SelS were transfected into cells by Lipofectamine 2000.After 24 hours, the expression of green fluorescent protein was observed and transfection efficiency was detected by FACS analysis. We collected the cells to isolate the total RNA by TRIzol method, and then cDNA was obtained by mRNA reverse transcription and amplified by PCR.Results The sequencing results showed that the recombinant genes were completely the same as the target genes, indicating that we constructed the plasmids successfully. The expression of green fluorescent protein could be observed and transfection efficiency was detected up to 40% by FACS analysis.PCR results showed that the target selenoprotein gene was highly expressed in the experimental group than in control group. Conclusion The truncated and normal selenoprotein S genes were successfully constructed and transfected into cells where they were highly expressed. It lays foundation for observing the biological effect of truncated and normal selenoprotein in cell line or animal body.
